Donor and Honored Persons Information Form

The Foundation is honored to administer a wide variety of funds created by donors to support the charitable needs of our region. We like to gather information about those who have created funds at the Foundation and those in whose memory or honor funds have been created. We encourage fund creators to complete the Donor and Honored Persons/Organization Information Form. We will use this form to create a brief profile about your fund on our web site.
